
Module 1 - Introduction
Acute & Long Term Care
At my Acute Care sites, I worked with my preceptors and Registered Dietitians at both Guthrie Lourdes Hospital and Wilkes-Barre General Hospital. These facilities are acute care hospitals that serve diverse patient populations and provide a wide range of clinical services. During my rotation, I collaborated with dietitians as part of the interdisciplinary healthcare team, gaining experience in clinical nutrition care, documentation, and patient-centered interventions. At these locations, dietitians utilized electronic medical records systems to assess, monitor, and manage patient care.
At my Long Term Care site, I worked with my preceptor and Registered Dietitian at Oak Ridge Rehabilitation and Healthcare Center. This facility provides skilled nursing, rehabilitation, and long-term care services for residents. During my rotation, I participated in nutrition assessments, care planning, and monitoring of residents with chronic conditions. I also gained experience working with the interdisciplinary team to support individualized care plans and improve residents’ overall quality of life. At this facility, I used an electronic medical record system to document and track patient progress
In my Medical Nutrition Therapy rotations, I completed a series of modules that reflect the progression of my clinical knowledge and professional development. Module 2 focuses on ethics and professional development.
Module 3 highlights nutritional screening and assessment, where I developed skills in evaluating patients and identifying nutrition-related risks.
In Module 4, I present my case study project, which demonstrates my ability to apply the Nutrition Care Process in a clinical setting.
Module 5 emphasizes nutrition education and counseling, where I practiced developing individualized interventions and communication strategies.
My Long Term Care and Pediatrics experiences are included in Module 6, showcasing my ability to adapt nutrition care across different populations.
In the final module, I provide an overview of my professional practice rotation at Guthrie Lourdes Hospital and Wilkes-Barre General Hospital. All of my projects and work are presented throughout these modules to reflect my growth and competency in medical nutrition therapy.
